Welcome to Extend Home Health Care. These Terms and Conditions (“Terms”) govern your use of our website, services, and participation in our programs. By accessing or using our website or services, you agree to comply with these Terms. If you do not agree, please do not use our services.
1. Definitions
“Provider,” “we,” “us,” and “our” means Extend Home Health Care.
“Participant,” “you,” and “your” means the person receiving services or using our website.
“Services” means all supports, programs, and assistance provided by Extend Home Health Care under the NDIS or otherwise.
2. Use of Website and Services
You agree to use our website and services lawfully and respectfully. You will not:
Use the website or services for any unlawful or fraudulent purpose.
Interfere with or disrupt the security or functioning of our systems.
Attempt to access data or accounts without authorisation.
We reserve the right to restrict or terminate your access if you breach these Terms.
3. Service Eligibility and Agreements
Services are available only to eligible NDIS participants or other clients as determined by us. Before service commencement, a written service agreement outlining supports, fees, responsibilities, and other terms will be provided. By signing this agreement, you accept these Terms and agree to:
Provide accurate and complete information relevant to your care.
Notify us promptly of any changes to your circumstances or NDIS plan.
Comply with the agreed service schedules and payment terms.
4. Privacy and Confidentiality
We are committed to protecting your privacy in accordance with the Privacy Act 1988 (Cth) and Australian Privacy Principles. Your personal and health information will be collected, stored, and used only for purposes related to service delivery, quality improvement, and compliance with legal obligations. 5. Fees and Payment
Fees for services will be outlined in your service agreement and are generally funded by the NDIS or other sources. You agree to pay any fees that are your responsibility promptly. We reserve the right to charge cancellation fees for missed appointments without adequate notice, as detailed in our Cancellation Policy.
6. Cancellation and Rescheduling
Please provide at least 24 hours’ notice if you need to cancel or reschedule a booked service. Late cancellations or no-shows may incur fees. Repeated cancellations without valid reasons may lead to service suspension or termination.
7. Participant Responsibilities
To ensure safe and effective service delivery, you agree to:
Follow all health and safety instructions provided by our staff.
Treat our staff and other participants with respect and dignity.
Provide a safe environment for support workers to deliver services.
Inform us of any health, safety, or behavioural issues that may affect service delivery.
8. Provider Responsibilities
Extend Home Health Care commits to:
Delivering services in a professional, respectful, and culturally safe manner.
Ensuring all staff are appropriately qualified, trained, and compliant with NDIS Practice Standards.
Maintaining open communication and involving you in decisions about your supports.
Protecting your privacy and confidentiality.
9. Limitation of Liability
To the extent permitted by law, Extend Home Health Care excludes all liability for any loss, damage, injury, or expense arising from your use of our services or website, except where caused by our negligence or breach of statutory duty.
10. Intellectual Property
All content on our website, including text, graphics, logos, and images, is owned by Extend Home Health Care or used under license. You may not reproduce, distribute, or create derivative works without prior written permission.
11. Complaints and Dispute Resolution
We encourage you to raise any concerns or complaints directly with us so we can work toward a resolution. Our Feedback and Complaints Policy outlines the process for lodging and managing complaints. If unresolved, you may escalate your complaint to the NDIS Quality and Safeguards Commission.
12. Changes to Terms
We may update these Terms from time to time to reflect changes in legal requirements, services, or operational needs. Updated Terms will be posted on our website with a new effective date. Continued use of our services after changes constitutes acceptance.
13. Governing Law
These Terms are governed by the laws of Western Australia and the Commonwealth of Australia. Any disputes will be subject to the exclusive jurisdiction of courts in Western Australia.
